Abstract BACKGROUND: Brazil’s Family Health Strategy is based on a primary healthcare model, which is considered to have case resolution capacity, with physicians at its center. OBJECTIVES: To evaluate the levels of occupational stress and work engagement among primary healthcare physicians. DESIGN AND SETTING: Cross-sectional study conducted in 2017, in São José do Rio Preto, São Paulo, Brazil. METHODS: A non-probability sample including 32 physicians from family health teams was used. Three self-applied instruments were used: a scale developed by the researchers seeking sociodemographic and professional variables, the Work Stress Scale and the Utrecht Work Engagement Scale. RESULTS: Female professionals (59.4%), permanent employees (56.3%), workload of 40 hours per week (59.4%) and 3-10 years of acting in primary care (68.8%) were more prevalent. Six professionals (19.4%) exhibited significant stress (score ≥ 2.5). The main stressors were lack of prospects for career growth (2.9 ± 1.3), form of task distribution (2.7 ± 1.0), poor training (2.7 ± 1.2) and insufficient time to perform the job (2.6 ± 1.2). Levels of work engagement ranged from 4.3 to 4.6 and were rated as high in all dimensions. Physicians with occupational stress had average levels of work engagement, whereas those without occupational stress had high levels of work commitment. CONCLUSIONS: A notable percentage of the physicians were experiencing occupational stress. The physicians had high levels of work engagement. Occupational stress was negatively correlated with work engagement, and it significantly compromised physicians’ levels of work engagement and interfered with their positive relationship with the work environment.

Abstract Genetic changes in platelet serotonin receptors (5-HTR2A) impair the initial process of tissue repair, regardless of the triggering factor of the skin wound. Objective was to determine the prevalence of the 102T-C polymorphism in the 5-HTR2A gene in Brazilian patients with and without skin wounds. Cross-sectional case-control study, in which 100 patients were evaluated as Cases Group (subdivided into I-with Chronic Wound and II-with Acute Wound) and 100 individuals as Controls, of both genders. DNA was extracted from leukocytes of peripheral blood and the region that covers the polymorphism was amplified by the molecular techniques Polymerase Chain Reaction/Restriction Fragment Length Polymorphism. The TT genotype was significantly associated with the protective factor against alterations in the healing process of skin wounds (OR: 0.4833; 95%CI: 0.2704-0.8638; p<0.05) in the Control Group. The genotypic analysis between Cases Group (I-Chronic Wound and II-Acute Wound) determined that the TT genotype was significantly associated with the protection factor in Case II (OR: 0.3333; 95%CI: 0.1359-0.8177; p<.005) and the CC genotype was significantly associated with the chance to develop chronic ulcers in the Case I (OR: 6.667; 95%CI: 1.801-24.683; p<0.05). Patients with chronic skin wounds have a higher prevalence of the 102T-C polymorphism in the 5-HTR2A gene, which is associated to alterations in the healing process in this population. There are differences, at the molecular level, in patients, with and without these lesions, and the probable role of the serotonergic system in wound healing.

Resumo Introdução: Desvio do septo e/ou da pirâmide nasal em crianças, se não tratado, pode apresentar problemas estéticos ou funcionais após anos, devendo ser corrigido o quanto antes. Objetivo: Relatar experiência cirúrgica no tratamento de crianças com desvio de septo nasal e/ou pirâmide nasal. Método: Revisão de prontuários de 202 crianças, 124 (61,4%) do gênero masculino e 78 (38,6%) do feminino, entre quatro e 16 anos (M = 11 anos), submetidas a rino e/ou septoplastia de janeiro de 1994 a janeiro de 2010, no Serviço de Otorrinopediatria do Departamento de Otorrinolaringologia e Cirurgia de Cabeça e Pescoço. Resultados: Septoplastia feita em 157 casos (77,7%); rinosseptoplastia em 23 casos (11,4%) e rinoplastia em 22 casos (10,9%). Conclusão: Alterações nasais devem ser corrigidas em crianças, para proporcionar crescimento harmônico e evitar as graves sequelas encontradas no respirador bucal.

Abstract Introduction: Untreated septal and/or nasal pyramid deviation in children should be corrected as soon as possible, because they can result in esthetic or functional problems years later. Objective: To report the surgical experience in treating children with nasal septum and/or nasal pyramid deviation. Methods: Review of medical records of 202 children, 124 (61.4%) males and 78 (38.6%) females, between 4 and 16 years of age (M = 11 years) who underwent rhinoplasty and/or septoplasty in a Pediatric Otolaryngology Service of the Dept. of Otolaryngology and Head and Neck Surgery between January 1994 and January 2010. Results: Septoplasty performed in 157 cases (77.7%); rhinoseptoplasty in 23 cases (11.4%), and rhinoplasty in 22 cases (10.9%). Conclusion: Nasal changes should be corrected in children, in order to provide harmonious growth, and prevent severe sequelae found in mouth breathers.

Introdução: A complexidade das estruturas da ponta nasal e o impacto de manobras cirúrgicas sobre o seu suporte dificultam a previsão da forma final da mesma. Devido a isso, nenhuma técnica isolada é suficiente para corrigir adequadamente as numerosas apresentações anatômicas, sendo o planejamento pré-operatório, a base da rinoplastia. Objetivos: Apresentar resultados de rinoplastias, por meio da abordagem cirúrgica gradativa para definição da ponta nasal baseada nas características anatômicas, e avaliar o grau de satisfação dos pacientes após a realização do procedimento cirúrgico. Método: Estudo em coorte histórica longitudinal no qual foram avaliados os prontuários de 533 pacientes de ambos os gêneros submetidos à rinoplastia no período de Janeiro de 2005 a Janeiro de 2012. Os pacientes foram divididos em sete grupos: (1) Nenhuma cirurgia na ponta nasal;( 2) Divulsão interdomal; (3) Ressecção cefálica; (4) Sutura domal; (5) Enxerto em escudo; (6) Divisão vertical dos domus; (7) Reconstrução das cartilagens alares maiores. Resultados: O grupo 4 foi o de maior prevalência. A taxa de satisfação foi de 96% e a revisão cirúrgica ocorreu em 4% dos casos. Conclusão: O protocolo utilizado permitiu a associação da abordagem cirúrgica gradativa para definição da ponta nasal com as características anatômicas nasais, alta taxa de satisfação como resultado cirúrgico e baixa taxa de revisão.

Introduction: The complexity of the nasal tip structures and the impact of surgical maneuvers make the prediction of the final outcome very difficult. Therefore, no single technique is enough to correct the several anatomical presentations, and adequate preoperative planning represents the basis of rhinoplasty. Objective: To present results of rhinoplasty, through the gradual surgical approach to nasal tip definition based on anatomical features, and to evaluate the degree of patient satisfaction after the surgical procedure. Methods: Longitudinal retrospective cohort study of the medical charts of 533 patients of both genders who underwent rhinoplasty from January of 2005 to January of 2012 was performed. Cases were allocated into seven groups: (1) no surgery on nasal tip; (2) interdomal breakup; (3) cephalic trim; (4) domal suture; (5) shield-shaped graft; (6) vertical dome division; (7) replacement of lower lateral cartilages. Results: Group 4 was the most prevalent. The satisfaction rate was 96% and revision surgery occurred in 4% of cases. Conclusion: The protocol used allowed the implementation of a gradual surgical approach to nasal tip definition with the nasal anatomical characteristics, high rate of patient satisfaction with the surgical outcome, and low rate of revision.

Evidências epidemiológicas sugerem que variantes genéticas que codificam enzimas envolvidas no metabolismo do folato podem modular o risco de câncer de cabeça e pescoço por alterar a metilação, síntese de DNA e estabilidade genômica. OBJETIVOS: Realizar uma revisão bibliográfica sobre polimorfismos genéticos envolvidos no metabolismo do folato e o risco de câncer de cabeça e pescoço. METODOLOGIA: Realizou-se uma busca eletrônica na base de dados Medline, selecionando estudos em câncer de cabeça do pescoço e polimorfismos envolvidos no metabolismo do folato. RESULTADOS: A associação do polimorfismo MTHFR C677T no risco dessa neoplasia foi avaliada em nove estudos e três deles mostraram associação com essa doença. Os polimorfismos MTR A2756G e MTRR A66G e RFC1 A80G também foram associados com aumento de risco para o câncer de cabeça e pescoço. O polimorfismo MTHFD1 G1958A não mostra associação com o risco dessa doença e os resultados da avaliação do polimorfismo MTHFR A1298C nesse tipo de neoplasia são contraditórios. Outros polimorfismos envolvidos no metabolismo do folato ainda não foram estudados nesse tipo de neoplasia. CONCLUSÃO: Concluímos que polimorfismos envolvidos no metabolismo do folato podem modular o risco desse tipo de tumor, no entanto, esses resultados precisam ser comprovados em diferentes populações.

Epidemiological evidence suggests that genetic variants encoding enzymes involved in folate metabolism may modulate HNSCC risk by altering DNA methylation synthesis and genomic estability. AIM: A review of the literature on genetic polymorphisms involved in folate metabolism and risk of head and neck cancer was carried out. METHODOLOGY: An electronic search was made on the Medline database to select papers on head and neck cancer and polymorphisms involved in folate metabolism. RESULTS: The association between MTHFR C677T polymorphism and the risk of this tumor type was evaluated in nine studies; there was an association with this disease in three papers. The MTR A2756G and MTRR A66G and RFC1 A80G polymorphisms were also associated with increased risk for HNSCC. MTHFD1 G1958A polymorphism was not associated with increased risk of this disease; the evaluation results of the MTHFR A1298C polymorphism in this neoplasm were contradictory. Other polymorphisms involved in folate metabolism were not studied for this neoplasm. CONCLUSION: We conclude that polymorphisms involved in folate metabolism may modulate the risk of head and neck cancer, however, these results need to be demonstrated in different populations.

CONTEXTO E OBJETIVOS: O câncer de cabeça e pescoço é o quinto tipo mais comum entre todas as neoplasias no mundo. O objetivo do estudo foi avaliar os parâmetros clínicos e epidemiológicos em um serviço de cirurgia de cabeça e pescoço. TIPO DE ESTUDO E LOCAL: Estudo transversal com coleta de dados de prontuários, realizado no departamento de otorrinolaringologia e cabeça e pescoço de um hospital universitário do noroeste do estado de São Paulo. MÉTODOS: Um total de 995 pacientes do serviço de cirurgia de cabeça e pescoço foi avaliado entre janeiro de 2000 a maio de 2010. As variáveis analisadas foram: idade, gênero, cor da pele, consumo de álcool e tabaco, sítio primário, estádio e tipo histológico do tumor, tratamento e número de mortes. RESULTADOS: A doença foi mais frequente entre homens (79,70%), tabagistas (75,15%) e etilistas (58,25%). Os locais mais representativos foram: cavidade oral (29,65%) e laringe (24,12%) para sítio primário; carcinoma espinocelular (84,92%) foi o tipo histológico mais frequente e cirurgia (29,04%) e radioterapia (14,19%) foram os tratamentos mais comuns. CONCLUSÃO: O câncer que afeta os pacientes assistidos pelo serviço de cirurgia de cabeça e pescoço ocorre, em sua maioria, entre homens, tabagistas e etilistas, tendo a cavidade oral e a laringe maior incidência. A alta taxa de pacientes com estádios III e IV indica diagnóstico tardio pelos centros de tratamento, o que reflete a necessidade de campanhas de prevenção para o diagnóstico precoce da doença.

CONTEXT AND OBJECTIVES: Head and neck cancer is the fifth most common type of cancer worldwide. The objective of this study was to evaluate the clinical and epidemiological parameters in a head and neck surgery service. DESIGN AND SETTING: Cross-sectional study using patients' records, developed in otolaryngology and head and neck department of a university hospital in the northwest of the state of São Paulo. METHODS: A total of 995 patients in the head and neck surgery service between January 2000 and May 2010 were evaluated. The variables analyzed included: age, gender, skin color, tobacco and alcohol consumption, primary site, staging and histological tumor type, treatment and number of deaths. RESULTS: The disease was more frequent among men (79.70%), smokers (75.15%) and alcohol abusers (58.25%). The most representative sites were oral cavity (29.65%) and larynx (24.12%) for the primary site; squamous cell carcinoma (84.92%) was the most frequent histological type, and surgery (29.04%) and radiotherapy (14.19%) were the most common treatments. CONCLUSION: The cancer that affects patients assisted by the head and neck surgery service occurs mainly men, smokers and alcohol abusers, and the oral cavity and larynx are the sites with the highest incidence. The high rate of patients with stages III and IV indicates late diagnosis by the treatment centers, which reflects the need for prevention education campaigns for early diagnosis of the disease.

A síndrome da apneia obstrutiva do sono (SAOS) é um dos distúrbios mais complexos do sono, envolvendo múltiplos fatores genéticos contribuintes para o fenótipo. A serotonina (5-HT) está envolvida na regulação de uma variedade de funções viscerais e fisiológicas, inclusive o sono. Polimorfismos no gene 5-HTR2A podem alterar a transcrição, afetando o número de receptores do sistema serotoninérgico, contribuindo para a SAOS. OBJETIVO: Investigar a prevalência dos polimorfismos T102C e -1438G/A no gene HTR2A em pacientes com e sem SAOS. MATERIAL E MÉTODO: Estudo molecular em 100 pacientes como casos-índice e em 100 como grupo controle, de ambos os gêneros. O DNA foi extraído de leucócitos de sangue periférico e realizada a amplificação das regiões que abrangem ambos os polimorfismos pelas técnicas da PCR-RFLP. DESENHO DO ESTUDO: Estudo de caso/controle em corte transversal. Resultados: Houve prevalência significativa do gênero masculino nos casos-índice em relação aos controles (p<0,0001). Para o polimorfismo T102C, não houve diferença genotípica significante entre casos e controles (p=1,000). Houve diferença significativa entre o genótipo AA do polimorfismo -1438G/A e pacientes com SAOS (OR:2,3; IC95%:1,20-4,38; p=0,01). CONCLUSãO: Os mecanismos serotoninérgicos parecem estar relacionados a SAOS. Não há diferenças na prevalência do polimorfismo T102C entre os pacientes com SAOS e o grupo controle. Há evidências de associação entre o polimorfismo -1438G/A e a SAOS.

Obstructive sleep apnea syndrome (OSAS) is one of the most complex disorders of sleep; it involves several genetic factors that contribute to the phenotype. Serotonin (5-HT) regulates a variety of visceral and physiological functions, including sleep. Gene 5-HTR2A polymorphisms may change the transcription of several receptors in the serotoninergic system, thereby contributing to OSAS. AIM: To investigate the prevalence of T102C and -1438G/A polymorphisms in the 5-HTR2A gene of patients with and without OSAS . MATERIAL AND METHOD: A molecular study of 100 index-cases and 100 controls of both genders. DNA was extracted from blood leukocytes samples and the regions that enclose both polymorphisms were amplified with PCR-RFLP. STUDY DESIGN: A cross-sectional case study. RESULTS: There was a significant prevalence of males in index cases compared to controls (p<0.0001). No significant genotypic differences between cases and controls were found in T102C polymorphisms (p=1.000).There were significant differences between the AA genotype of -1438G/A polymorphisms and patients with OSAS (OR:2.3; CI95%:1.20-4.38, p=0.01). CONCLUSION: Serotonergic mechanisms may be related to OSAS. There were no differences in the prevalence of T102C polymorphisms in patients with OSAS and the control group. There is evidence of an association between the -1438G /A polymorphism and OSAS.

Homens com síndrome da apneia obstrutiva do sono (SAOS) podem apresentar diminuição dos níveis de testosterona devido à hipóxia. OBJETIVOS: Relacionar os níveis séricos da testosterona, em pacientes com SAOS, com parâmetros clínico-laboratoriais. MATERIAL E MÉTODOS: Foram revisados 103 prontuários de pacientes com SAOS, entre os anos de 2002 e 2009, e coletados os seguintes dados: idade à época da realização da polissonografia, valores do Hematócrito e Hemoglobina, nível sérico da testosterona total, IMC, índice de apneia/hipopneia(IAH) e SatO2. FORMA DO ESTUDO: Estudo de casos retrospectivo em corte transversal. RESULTADOS: 79 pacientes (77%) não apresentaram alteração hormonal e 24 (23%) apresentaram níveis séricos inferiores. Dos pacientes com testosterona normal 70% estavam com sobrepeso, enquanto que 63% com testosterona alterada apresentaram obesidade grau I (p<0,05). Os pacientes com testosterona alterada apresentaram as dosagens médias do Ht e da Hb e dos níveis médios do andrógeno significantemente inferiores aos dos pacientes sem alteração androgênica. A média do IMC dos pacientes com alteração hormonal foi significativamente maior que a média daqueles sem alteração. CONCLUSÃO: A relação entre o perfil sérico da testosterona matinal e a obesidade e, em menor grau, a idade, o IAH e a hipóxia, podem ser responsáveis pela supressão central da testosterona nesses pacientes. A queda dos valores hematimétricos pode ser relacionada aos baixos níveis circulantes da testosterona.

Males with obstructive sleep apnea syndrome (OSAS) may present decreased testosterone serum levels because of hypoxemia. AIM: To correlate testosterone levels in OSAS patients with laboratory parameters. MATERIAL AND METHODS: 103 registries of OSAS patients were reviewed from 2002 to 2009. The following data collected: age when polysomnography was done, hematocrit and hemoglobin levels, total testosterone serum levels, BMI, apnea/hypopnea index (AHI), and O2 saturation. STUDY DESIGN: A cross-sectional retrospective case study. RESULTS: 79 patients (77%) had no hormonal changes, and 24 patients (23%) had decreased serum levels. In patients with normal testosterone levels, 70% were overweight; 63% with altered testosterone levels had obesity grade I (p<0.05). Patients with altered testosterone levels had significantly lower average doses of Ht, Hb and androgen compared to patients without altered androgen levels. The average BMI of patients with altered hormone levels was significantly higher compared to patients with normal hormone levels. CONCLUSIONS: The relationship between morning testosterone levels and obesity, and to a lesser degree age, AHI and hypoxemia may be the cause of central suppression of testosterone in these patients. Decreased blood HT and HB levels may be related to lower levels of circulating testosterone.

INTRODUÇÃO: A otorrinolaringologia é uma especialidade médica que abrange 4,4% do total de médicos paulistas, tendo uma variedade de procedimentos cirúrgicos distribuídos entre faringe, nariz, ouvido e laringe. O conhecimento do perfil de um serviço de referência em otorrinolaringologia permite a sua melhor organização, dimensionamento do volume do atendimento e das cirurgias realizadas, proporcionando uma melhor formação ao aluno e ao médico residente. OBJETIVO: Descrever o perfil das cirurgias nasossinusais e dos pacientes submetidos a elas no departamento de otorrinolaringologia e cirurgia de cabeça e pescoço de um hospital escola. MÉTODO: Foi realizado estudo de coorte histórico com corte transversal retrospectivo, com revisão de 872 prontuários de pacientes submetidos a cirurgias rinossinusais entre janeiro de 2006 e dezembro de 2008. Utilizado questionário próprio, buscando sexo, idade, diagnóstico cirúrgico e cirurgia realizada. RESULTADOS: Do total de 872 pacientes analisados, 45,4% eram sexo feminino e 54,6% sexo masculino, variando em uma faixa etária 4 a 80 anos (média de 29,8 anos). Os principais diagnósticos cirúrgicos foram: desvio septo nasal(n=457), deformidade nasal pós-trauma (n=287), hipertrofia de conchas nasais (n=153), polipose rinossinusal(n=73), rinossinusite crônica(n=32). Entre os procedimentos cirúrgicos mais frequentemente realizadas, estão: septoplastia (n=388), rinosseptoplastia (n=215), FESS (n=131), cauterização intra-turbinal(n=114), rinoplastia reparadora(n=73), turbinectomia(n=43), turbinoplastia (n=55). Ressalta-se que os pacientes podem ter recebido mais de um diagnóstico cirúrgico e realizado mais de uma cirurgia, dependendo da indicação. CONCLUSÃO: Apresenta-se o volume e a diversidade de cirurgias rinossinusais realizadas em nosso serviço, contribuindo com a escassa produção científica sobre esse tipo de casuística.

INTRODUCTION: The ENT is a medical specialty that covers 4.4% of all doctors in São Paulo, with a variety of surgical procedures distributed between pharynx, nose, ear and larynx. Knowing the profile of a reference service in otolaryngology allows for a better organization, scaling the volume of care and surgeries, providing better training to the student and resident physician. OBJECTIVE: To describe the profile of sinonasal surgery and patients to them in the department of otolaryngology and head and neck surgery at a teaching hospital. METHOD: We conducted a cohort study of cross-sectional retrospective study with review of 872 charts of patients undergoing surgery Sinus between January 2006 and December 2008. Used questionnaires, seeking sex, age, surgical diagnosis and surgery. RESULTS: Of 872 patients studied, 45.4% were female and 54.6% male, ranging in an age group 40-80 years (mean 29.8 years). The main surgical diagnoses were: nasal septum deviation (n=457), nasal deformity after trauma (n=287), enlarged turbinates (n=153), rhinosinusal polyposis (n=73), chronic sinusitis (n=32). Among the most frequently performed surgical procedures include: septoplasty (n=388), rhinoplasty (n=215), FESS (n=131), intra-turbinal cauterization (n=114), reconstructive rhinoplasty (n=73), turbinectomy (n=43), turbinoplasty (n=55). It is emphasized that patients may have received more than one surgical diagnosis and realized more than one surgery, depending on the alert. CONCLUSION: We present the volume and diversity of Sinus surgeries performed in our department, contributing to the scarce scientific literature on this type of case.

O polimorfismo C677T do gene metilenotetra-hidrofolato redutase (MTHFR) pode ser um fator de risco para o carcinoma espinocelular de cabeça e pescoço devido a alterações nos níveis de folato que podem induzir alterações na metilação intracelular, promovendo a carcinogênese. OBJETIVO: Avaliar o polimorfismo MTHFR C677T em pacientes com carcinoma espinocelular de cabeça e pescoço e em indivíduos sem história de neoplasia e verificar a associação desta doença com as características clínico-patológicas. CASUÍSTICA E MÉTODOS: Estudo retrospectivo no qual foram avaliados gênero, idade, tabagismo, etilismo e parâmetros clínico-histopatológicos em 200 indivíduos (100 com a doença e 100 sem história de neoplasia). A análise molecular foi realizada pela técnica de PCR- RFLP e os testes qui-quadrado de Pearson e Regressão Logística Múltipla foram utilizados para análise estatística. RESULTADOS: Não houve associação entre o polimorfismo MTHFR C677T e a doença (p=0,50). Diferenças significantes entre o grupo de pacientes e o grupo controle foram observadas para idade superior a 50 anos, hábito tabagista e gênero masculino (p<0,001). Não houve associação da doença com os parâmetros clínico-histopatológicos. CONCLUSÃO: Não foi possível estabelecer uma associação entre o polimorfismo MTHFR C677T e o carcinoma espinocelular de cabeça e pescoço.

Methylenetetrahydrofolate reductase gene (MTHFR) C677T polymorphism may be a risk factor for head and neck squamous cell carcinoma due to changes in folate levels that can induce disorders in the methylation pathway, which results in carcinogenesis. AIM: To evaluate MTHFR C677T polymorphism in patients with head and neck squamous cell carcinoma and in individuals with no history of cancer, and to assess the association of this disease with clinical histopathological parameters. SERIES AND METHODS: A retrospective study that assessed gender, age, tobacco, alcohol consumption and clinical histopathological parameters in 200 patients (100 with disease and 100 with no history of cancer). PCR-RFLP molecular analysis was carried out and the chi-square test and multiple logistic regression were applied for the statistical analysis. RESULTS: There was no association between MTHFR C677T polymorphism and head and neck cancer (p = 0.50). Significant differences between the study and control groups were observed at age over 50 years, tobacco use, and male gender (p <0.001). There was no association of disease with clinical-histopathological parameters. CONCLUSION: No association between the MTHFR C677T polymorphism and head and neck squamous cell carcinoma was possible in this study.

Traumas faciais são frequentes em emergências requerendo o diagnóstico de fraturas e lesões associadas. OBJETIVO: Avaliar dados epidemiológicos de atendimento em trauma facial. MATERIAL E MÉTODOS: Foram revisados 335 prontuários de pacientes com trauma facial tratados pelo Serviço de Otorrinolaringologia, no período de Janeiro de 2002 a Dezembro de 2008. Os seguintes dados foram coletados: idade, gênero, etiologia, local anatômico da fratura, lesão associada, consumo de álcool, tratamento e hospitalização. FORMA DO ESTUDO: Estudo de casos retrospectivo em corte longitudinal histórico. RESULTADOS: A maioria dos pacientes são homens adultos jovens (p<0,005) com uma proporção masculino:feminino de 4:1 (p<0,05). Violência interpessoal é a causa mais prevalente de trauma facial (27,9%) seguida de acidente automobilístico (16,6%) (p<0,05). Mandíbula é o osso facial fraturado mais prevalente (44,2%) seguido pela fratura nasal (18,9%) (p<0,05). Houve consumo de álcool em 41,1% dos pacientes com uma proporção masculino:feminino de 11,2:1 (p<0,05). Setenta e sete por cento dos pacientes necessitaram de intervenção cirúrgica (p<0,05) e 84,5% foram hospitalizados (p<0.05). CONCLUSÃO: Homens adultos jovens são as vítimas mais prevalentes em trauma facial e a violência interpessoal é a responsável pela maioria das lesões faciais. A maioria dos casos de traumatismo facial está associada ao consumo de álcool. Estudos posteriores serão sempre necessários a fim de permitir uma clara compreensão da tendência na etiologia do trauma facial.

Facial traumas are frequent in emergencies, and they require the diagnosis of fractures and associated lesions. AIM: To analyze epidemiological data concerning facial trauma care. MATERIALS AND METHODS: Three hundred and fifty-five charts from patients with facial trauma treated by the Service of Otorhinolaryngology, from January 2002 to December 2008, were revised. The following data was collected: age, gender, etiology, anatomical localization of the fracture, associated injuries, alcohol consumption, treatment, and hospitalization. STUDY DESIGN: A retrospective historical longitudinal study. RESULTS: Most of the patients are young adult men (p<0.05) with a male:female ratio of 4:1(p<0.05). Interpersonal violence is the most prevalent cause of facial trauma (27.9%), followed by motor vehicle accidents (16.6%) (p<0.05). The mandible is the most prevalent facial bone fractured (44.2%), followed by nasal fracture (18.9%) (p<0.05). 41.1% of the patients consumed alcohol with a male:female ratio of 11.2:1 (p<0.05). Seventy-seven percent of the patients required surgical intervention (p<0.05) and 84.5% were hospitalized (p<0.05). CONCLUSION: Young male adults are the most prevalent victims of facial trauma, and interpersonal violence is responsible for the majority of the facial injuries. Most of the cases of facial trauma are associated with the consumption of alcohol. Further studies will be necessary to provide a clear understanding of the trends in the etiology of facial trauma.

As Cirurgias Ortognáticas apresentam grande importância para a correção das anomalias dentofaciais e tratamento da Síndrome da Apneia e Hipopneia do Sono - SAHOS. Atualmente observa-se que grande parte da população apresenta algum tipo de desvio morfológico e/ou funcional do sistema estomatognático. OBJETIVO: O presente trabalho visa um estudo do perfil dos indivíduos que foram tratados no Serviço de Cirurgia Ortognática de uma escola médica. MATERIAL E MÉTODO: Pesquisa em prontuários - 2004 a 2008 - identificação de procedimentos, características dos indivíduos, maloclusão e cirurgia realizada. RESULTADOS: Esse trabalho mostrou que o número de tratamentos cirúrgicos para a correção das deformidades vem aumentando. As cirurgias de avanço de maxila exclusivamente correspondem ao maior número da amostra. CONCLUSÃO: Tem havido aumento dos casos de cirurgia ortognática nos últimos anos e os avanços de maxila exclusivamente corresponderam ao maior número dos tratamentos cirúrgicos.

Orthognathic surgeries are very important for both the correction of dentofacial deformities as well as for the treatment of obstructive sleep apnea/hypopnea syndrome. Nowadays, most of the population presents some type of morphological and/or functional disorder of the stomatognathic system. AIM: The present study aims at assessing the information from the individuals treated in the Orthognathic Surgical Service of a Medical School. METHODS: Search in medical records - 2004-2008, identification procedures, individual characteristics, malocclusion and surgery. RESULTS: The number of surgical treatments due to dentofacial deformities has been increasing. Maxillary advancement surgery alone comprised the highest number in the sample. CONCLUSION: Orthognathic surgery cases have been increasing in the last years and maxillary advancement alone comprised the highest number of surgical treatments.

No Brasil foram estimados 14.160 casos novos de câncer e cabeça e pescoço para o ano de 2008. O tabagismo e o etilismo são os principais fatores de riscos estabelecidos na etiologia dessa doença. OBJETIVO: Avaliar o polimorfismo T → A do gene TAX1BP1 (leu306ile) em pacientes com câncer de cabeça e pescoço e em uma população controle. CASUÍSTICA E MÉTODOS: Estudo retrospectivo onde foram avaliados o sexo, idade, tabagismo e etilismo de 191 pacientes com câncer de cabeça e pescoço e de 200 indivíduos sem história de neoplasia. A análise molecular foi realizada após extração de DNA genômico pela técnica de PCR-RFLP. RESULTADOS: Há predominância de pacientes do sexo masculino (84,82%), tabagistas (91,1%) e etilistas (77,49%). A avaliação molecular não mostrou diferença estatisticamente significante entre os dois grupos (p =0,32). A análise dos parâmetros clínicos e polimorfismos mostrou uma associação com câncer na cavidade oral (OR:2,38; IC 95%: 1,18-4,78; p = 0,01), os demais parâmetros não mostraram associação com o polimorfismo. CONCLUSÃO: Há evidências de associação entre o polimorfismo do gene TAX1BP1 e câncer de cavidade oral. Para os demais parâmetros analisados os resultados não sugerem uma associação com o polimorfismo do gene TAX1BP1.

In Brazil, there were 14,160 new estimated cases of head and neck cancer for the year of 2008. Smoking and drinking are the main risk factors established in the etiology of this disease. AIM: to assess the T → A polymorphism in gene TAX1BP1 (leu306ile) in patients with head and neck cancer and a control population. SERIES AND METHODS: a retrospective study in which we assessed the gender, age, smoking and drinking habits of 191 patients with head and neck cancer and 200 individuals without history of neoplasia. The molecular analysis was carried out after genomic DNA extraction by the PCR-RFLP method. RESULTS: there is a predominance of males (84.82%), smokers (91.1%) and drinkers of alcohol (77.49%). Molecular assessment did not show statistically significant differences between the two groups (p =0.32). The analysis of clinical parameters and polymorphisms showed association with oral cavity cancer (OR: 2.38; CI 95%: 1.18-4.78; p = 0.01), the other parameters were not associated with the polymorphism. CONCLUSION: There is evidence of association between TAX1BP1 gene polymorphism and oral cavity cancer. For the remaining parameters analyzed, the results do not suggest association with the TAX1BP1 gene polymorphism.

O angiofibroma nasofaríngeo juvenil (ANJ) é um raro tumor em adolescentes masculinos originário na nasofaringe. OBJETIVOS: Apresentar a experiência do tratamento do ANJ em pacientes do Serviço de Otorrinolaringologia entre 2001 e 2008. MATERIAL E MÉTODOS: Foram revisados de prontuários do Serviço de Otorrinolaringologia os dados demográficos, apresentação clínica, métodos de investigação e tratamento de 16 pacientes. DESENHO DO ESTUDO: estudo descritivo, retrospectivo, de corte transversal. Resultados: Todos os pacientes são do gênero masculino e a média de idade ao diagnóstico foi de 16,8 anos (variação de 9 a 23 anos), sendo mais de 56% deles com estádio II de Fisch. Embolização pré-operatória foi realizada em 10 pacientes (62,5%). Todos os 16 pacientes foram submetidos à ressecção cirúrgica. Dois pacientes (66,7%) que não foram submetidos à embolização pré-operatória necessitaram de transfusão sanguínea. O índice de recidiva foi de 43,75% e o índice de cura foi 93,75%. CONCLUSÕES: Embolização pré-operatória diminui a perda sanguínea intra-operatória. O índice de recidiva foi relacionado ao estadiamento avançado do tumor ao diagnóstico e à não-realização de embolização pré-operatória. Cirurgia associada à embolização pré-operatória são os principais tratamentos do ANJ. Todos os pacientes devem ter estudos de imagem pré-operatórios, especialmente tomografia computadorizada, para auxiliar no planejamento cirúrgico e no seguimento.

Juvenile nasopharyngeal angiofibroma (JNA) is a rare tumor in adolescent males. It originates in the nasopharynx. AIM: to present the experience of JNA management at an Otorhinolaryngology Service between 2001 and 2008. MATERIALS AND METHODS: Demographical data, clinical presentation, investigations as well as the treatment of sixteen JNA patients were reviewed and collected from medical records from the ORL Service. DESIGN: Cross-sectional, retrospective and descriptive study. Results: All JNA patients were male. The average age at diagnosis was 16.8 years (range 9-23 years). More than 56% of the patients were classified as Fisch II. Preoperative embolization was carried out in ten (62.5%) patients. All 16 patients were submitted to primary surgical resection. Two patients (66.7%) who didn't receive preoperative embolization required intraoperative blood transfusion. The overall recurrence rate was 43.75% and the cure rate was 93.75%. CONCLUSION: Preoperative embolization minimizes intraoperative blood loss. The recurrence rate was related to advanced tumoral stage at diagnostic and the lack of preoperative embolization. Surgery combined with preoperative embolization is the major treatment for JNA. All the patients should undergo preoperative imaging studies, especially CT, to assist in surgical planning and follow-up.
